About The Position

The RN Case Manager provides nursing care and case management for patients with terminal disease. The role includes assessing how the disease affects patients’ physical, psychosocial, and environmental needs and activities of daily living, implementing and updating individualized plans of care, and coordinating care with patients, caregivers, physicians, healthcare professionals, and the Inter-Disciplinary Group. The position also includes assigned on-call duties, timely documentation, and compliance with applicable laws, Medicare regulations, and personnel policies and procedures.

Requirements

Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
Current state Registered Nurse license.
Current driver’s license, reliable transportation, and ongoing valid and current auto liability insurance.

Nice To Haves

Two years of experience as an RN in a healthcare or clinical setting is preferred.

Responsibilities

Conduct initial and ongoing comprehensive assessments of the impact of terminal disease on patients’ physical, psychosocial, and environmental needs and activities of daily living, including grief risk and cultural and spiritual needs, and verbal and non-verbal factors.
Implement individualized plans of care and recommend revisions as necessary.
Collaborate on care with patients and caregivers, physicians, and healthcare professionals.
Attend Inter-Disciplinary Group (IDG) meetings; coordinate and participate in patient care planning and prioritization of needs; collaborate with the IDG to promote coordination of patient care.
Perform assigned on-call responsibilities and provide on-call service to patients and families.
Submit all documentation in a timely manner.
Perform other duties as required.
Ensure overall compliance with local, state, and federal laws, Medicare regulations, and established personnel policies and procedures.
