About The Position

Amedisys is seeking a full-time, day-shift RN Case Manager Hospice to provide hospice care to patients at home. The role performs patient assessments and collaborates with the care team to develop and implement individualized plans of care. The nurse maintains accurate, timely patient progress records, monitors pain and symptom management, teaches patients and caregivers appropriate care techniques, and communicates regularly with the clinical manager and care team. On-call responsibilities and other assigned duties are also part of the position.

The annual salary range is $95,000–$100,000. Compensation may vary based on geographic location, job-related knowledge, skills, and experience. Benefit eligibility may vary by position and shift status; participation in certain benefits requires enrollment in an Amedisys medical plan.

Requirements

• One year of nursing experience.
• Current RN license specific to the state(s) assigned for work.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.

Responsibilities

• Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
• Make referrals to other hospice care disciplines as needed, or document the rationale for not doing so.
• Develop, prepare, and maintain individualized patient care progress records accurately and on time, according to care center policies.
• Record pain and symptom management changes and outcomes as appropriate.
• Teach patients, families, and caregivers appropriate patient care techniques.
• Communicate regularly with the clinical manager and care team.
• Perform on-call responsibilities and provide on-call services to patients and families as assigned.
• Perform other duties as assigned.
