The position evaluates outcomes based on clinical, financial, and utilization data, defining the unit of care as the family and patient. The nurse demonstrates knowledge of hospice palliative care, hospice philosophy, and scope of services, recognizing cultural attitudes towards death and dying, and identifying the five stages of grieving. They explain advanced directives, clinical requirements, and available interdisciplinary services, utilizing independent nursing judgment. The role participates in the creation and implementation of the plan of care, using age-specific structured care methodologies for neonatal, pediatric, adolescent, adult, and/or geriatric patients. The nurse interviews potential residents, assesses them in compliance with regulations, and establishes and evaluates a service plan for each resident. Communication with patients, families, and the healthcare team is essential, requiring independence, assertiveness, and critical thinking. The ability to document clearly, solve problems, and work with little direction is necessary. The role incorporates teaching/learning principles, adapts teaching to age groups, and facilitates groups and implements projects when needed. Strong flexibility, organizational skills, and the ability to establish priorities are required. Knowledge of reimbursement for services provided is also necessary.
