About The Position

Hospice Case Managers are vital members of our care team, providing compassionate, skilled nursing support to patients and families during end-of-life care. As a Hospice RN Case Manager, you will oversee patient care, coordinate interdisciplinary support, and ensure comfort, dignity, and quality of life for every patient you serve. At Newport Hospice, we don't just hire nurses; we hire Clinical Directors of the Home. We are looking for an RN Case Manager who understands that they are the "Captain of the Ship" for every patient under their care. If you are a nurse who takes pride in clinical autonomy, rigorous documentation, and total patient advocacy, we want you on our team. In this position, you are not just making visits—you are managing the entire clinical ecosystem of your patients. You are the primary point of contact for the family and the central hub for the Interdisciplinary Group (IDG).

Requirements

  • Graduate of an accredited school of nursing.
  • Current California RN license (BSN preferred).
  • Certification in Hospice and Palliative Nursing desired.
  • Minimum of one year of professional nursing experience within the last three years, or a BSN degree.
  • Strong clinical judgment, communication, and documentation skills.
  • Compassionate and supportive attitude toward patients and families facing end-of-life care.
  • Ability to maintain patient confidentiality and professionalism at all times.
  • Proficiency in computer use and electronic medical record (EMR) documentation.
  • Reliable, well-maintained personal vehicle with valid California driver’s license, acceptable driving record, and proof of auto insurance.
  • Minimum 1–2 years of Hospice Case Management experience is required.
  • Must know the "Hospice Benefit" inside and out.
  • Deep understanding of CMS Conditions of Participation (CoPs) and California Title 22. Or willingness to learn!
  • Expert ability to prioritize acuity and manage a field schedule effectively.
  • Proficiency in EMR (e.g., HospiceMD, KanTime, MatrixCare) with an emphasis on "gold-standard" clinical narratives.
  • Must be able to speak professionally with physicians and compassionately with families.

Responsibilities

  • Lead and coordinate the care provided by your MSW, Spiritual Counselor (SC), and CHHA.
  • Ensure interventions align with your overall clinical strategy.
  • Work hand-in-hand with the MD, providing clinical assessments, suggesting interventions, and securing medication orders and treatment plans immediately.
  • Perform full-body physical and psychosocial assessments at every visit.
  • Identify changes in status before they become crises.
  • Develop, update, and maintain a compliant, individualized plan of care that reflects the patient’s goals and regulatory requirements.
  • Manage the entire medication profile, including reconciliation, pharmacy coordination, and symptom management (GIP/Continuous Care triggers).
  • Lead the discussion for your patients during IDG meetings, providing the team with clear, concise, and professional updates that meet all CMS and Title 22 standards.
  • Complete comprehensive nursing assessments and develop individualized plans of care in collaboration with patients and families.
  • Provide direct hospice nursing care to maintain the highest level of comfort and quality of life.
  • Manage and update care plans as patient needs change.
  • Communicate with physicians to obtain appropriate orders and report changes in patient condition.
  • Initiate interventions and evaluate outcomes to ensure optimal symptom management.
  • Educate patients, families, and caregivers about hospice care, medications, and symptom recognition.
  • Accurately document patient care and communicate updates to the interdisciplinary team (IDG).
  • Coordinate with external service providers as needed to meet patient and family needs.
  • Ensure availability of necessary medical equipment and supplies.
  • Assign and supervise Home Health Aides, completing supervisory visits every 14 days.
  • Serve as IDG coordinator, collaborating with all team members to ensure holistic care.
  • Participate in on-call rotation to provide 24/7 hospice coverage and support.
