This role provides care to assigned patients in a caring, safe, and efficient manner and provides leadership to staff. The position seeks to evaluate outcomes based on an integration among established clinical, financial and utilization data. It defines the unit of care as the family and patient, and demonstrates knowledge of hospice palliative care, hospice philosophy, and scope of services. The role requires recognition of different cultural attitudes and values towards death and dying, identification of the five stages of the grieving process, and explanation of advanced directives, clinical requirements, and available interdisciplinary services. The RN Case Manager utilizes independent nursing judgment when integrating healthcare, participates in the creation and implementation of the plan of care by the interdisciplinary group, and has knowledge of and utilizes appropriate age-specific structured care methodologies for neonatal, pediatric, adolescent, adult, and/or geriatric patients/residents. The position involves interviewing potential residents and assessing them in compliance with regulations, policy, and procedure, establishing a service plan for each resident, and evaluating it on an ongoing basis. The role requires the ability to communicate with patients/residents, family members, and the health care team, demonstrating independence, assertiveness, and critical thinking. Documentation must be clear, and problem-solving skills are essential. The individual must be self-motivated, work with little direction, and be able to incorporate teaching/learning principles, adapting teaching to the age group. Facilitating groups and implementing projects may be necessary. Strong flexibility and organizational skills are required, along with the ability to establish priorities. Knowledge of reimbursement for services provided is necessary.

Requirements

  • Graduate from an accredited nursing program, including, but not limited to, those accredited by the Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), or National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA).
  • Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state.
  • Obtains and subsequently maintains required department specific competencies and certifications.
  • Must possess a valid driver’s license.

Nice To Haves

  • Bachelor's degree in Nursing strongly preferred.
  • Minimum of two years’ clinical experience preferred.

  • Uses assessment information for admission and continued stay in the hospice community.
  • Establishes a service plan for each resident and evaluates on an on-going basis.
  • Communicates with patients/residents, family members and others on the health care team.
