RN Case Manager Hospice
About The Position
As an RN Case Manager, you will coordinate compassionate, individualized hospice care by assessing patients, partnering with the interdisciplinary care team, and developing plans that support each patient’s needs. The role is with Amedisys, part of the Optum family of businesses, which provides home health, hospice, and community-based services. The position includes assigned on-call responsibilities and services to patients and families. The employer offers development opportunities and a benefits package subject to eligibility requirements. External candidates are eligible for a $10,000 sign-on bonus.
Requirements
- Current RN license specific to the state or states assigned.
- Current CPR certification.
- Valid driver's license, reliable transportation, and liability insurance.
Nice To Haves
- At least 1 year of clinical RN experience.
Responsibilities
- Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
- Make referrals to other hospice care disciplines as needed, or document the rationale for not doing so.
- Develop, prepare, and maintain individualized patient care progress records accurately and on time, according to care center policies.
- Record pain and symptom management changes and outcomes as appropriate.
- Teach patients, families, and caregivers appropriate patient care techniques.
- Regularly communicate with the clinical manager and care team.
- Perform on-call responsibilities and provide on-call services to patients and families as assigned.
- Perform other duties as assigned.
