About The Position

This role involves providing case management services within the Extended Care Facility (ECF) setting. The RN Case Manager will collaborate with nursing facility staff to ensure effective caregiving partnerships and demonstrate sensitivity to the unique mission and cultural climate of each facility. The position requires strong clinical skills, independent work capability, and effective communication with patients, families, and team members.

Requirements

  • Graduate of an accredited school of registered nursing.
  • Currently licensed as an RN through the Ohio Board of Nursing.
  • Must be able to obtain a license in the state of Michigan.
  • Minimum 2 years of medical-surgical nursing experience.
  • Working experience charting in an EMR system.
  • Strong clinical background with knowledge of disease processes and pathways.
  • Clinical expertise related to critical thinking, assessment, problem-solving, and technical skills.
  • Ability to multitask.
  • Ability to work independently and within an interdisciplinary team.
  • Must be flexible and adaptable to change.
  • Demonstrates strong organizational, time management, and prioritization skills.
  • Must be a licensed driver with an automobile in good working condition and is insured in accordance with state and agency requirements.
  • Must be CPR certified according to the American Heart Association CPR standards.
  • Meets state requirements for maintaining professional license.

Nice To Haves

  • BSN preferred.
  • Previous experience in the nursing home environment and/or hospice is preferred.
  • Netsmart system experience preferred.
  • Knowledge of Medicare hospice regulations and Joint Commission regulations preferred.

  • Demonstrate awareness and sensitivity to the unique mission and cultural climate of each facility environment.
  • Communicate effectively with patients, families, and team members regarding care issues.
  • Work independently and within an interdisciplinary team.
  • Manage time, organize tasks, and prioritize effectively.
  • Understand and apply hospice philosophy, principles of death/dying, pain/symptom management, and adult teaching/learning principles.
  • Maintain professional license and CPR certification.
