About The Position

As the CIN Care Navigation RN, you will coordinate and optimize care transitions for ACO and patients transitioning from acute care to post-acute care and ultimately, home. You will track patients transitioning into post-acute care facilities to ensure continued quality care. You will work with post-acute providers to monitor and improve patient outcomes. Requirements

Associate or bachelor’s degree in nursing.
Knowledge of case management and community resources.
Ability to function independently and effectively in a fast-paced environment.
Strong written and verbal skills.
Ability to establish priorities and communicate effectively.
At least 2 years of previous case management, care navigation, or post-acute care experience.
Must have active and unencumbered RN in Ohio and Michigan - or be willing to obtain upon hire.

Nice To Haves

Master’s degree in nursing or other relevant healthcare field.
Microsoft Office competency
2 years experience
