About The Position
Cheer Home Care is seeking a compassionate RN Care Case Manager to provide high-quality care to older adults in their homes. This role involves collaborating with an interdisciplinary team to create and execute individualized care plans, offering continuous feedback to clients, their representatives, caregivers, and nurses. The RN Care Case Manager will work closely with the office team, requiring excellent communication and problem-solving abilities. This position serves as the primary care manager, coordinating care among caregivers, RNs, therapists, and external representatives.
Requirements
- Active California RN License
- Strong Computer Skills
- Ability to multitask, drive, take action
- Strong Organizational & Communication Skills
- Pass Live Scan Fingerprint Clearance
- Valid driver’s license and proof of insurance required
- Cell phone with Internet Access
- Proof of Negative TB results
- Willing to Work One Weekend a Month as Back-Up On Call

Responsibilities
