About The Position

San Diego’s St. Paul's PACE program (Program of All-Inclusive Care for the Elderly). This innovative program is for individuals 55 years or older, who are living with chronic illness, or disabilities and need coordinated medical care to continue living as independently as possible in their home and community. The healthcare teams at St. Paul’s PACE provide a hands-on approach coordinating medical, social, and home care services so individuals no longer need to manage their medical care alone. The typical schedule for the position is Monday - Friday, 8:00AM - 4:30PM. Job Summary: Under the supervision of a Director of Clinical Services, the In Home Services Supervisor oversees the day-to-day operations of the Home Care Services Program for St. Paul’s PACE. The position provides direct supervision to In Home Services CNA Supervisor who oversees the Certified Nursing Assistants (CNAs), Personal Care Attendants (PCA), and the In Home Services Assistant of the Home Care Services Department. The position provides supervision visits to home care staff in the field to evaluate skill and performance. The position assist Director of Clinical Services in policy formulation and program development and implementation. This position is responsible for ensuring compliance with annual budgets and financial objectives, company policies and procedures, and guidelines. The position ensures timely completion of Home Care Services. The position acts as the liaison between any contracted home health agency and St. Paul’s PACE and ensures compliance with staff competency requirements. The position maintains the participants’ medical record thorough documentation and communicate participants’ changes and progress to appropriate staff.

Requirements

  • Graduate of a school of professional nursing required.
  • Current California Registered Nurses License or Vocational Nurse License
  • CPR and First Aid certification required.
  • A minimum of one year’s experience in working with the frail or elderly population is required.
  • Shall have either training or related experience in the job assigned.
  • MUST BE ABLE TO CLEAR FINGERPRINTS THROUGH DOJ AND FBI IN ADDITION TO A DRUG SCREEN CLEARANCE.

Responsibilities

  • Oversees the performance and daily operations of in-home support staff, including caregivers, supervisors, and schedulers, ensuring effective service delivery.
  • Monitor the Interdisciplinary Team meetings daily to ensure that all concerns related to in-home services are addressed promptly.
  • Provides orientation and competency assessments and supervises caregivers, CNA supervisors, and schedulers to ensure adequate department staffing and staff performance. Performs supervisory visits to staff in the field.
  • Investigate medication errors in-home and collaborate with the Clinical Service Manager to develop corrective action plans.
  • Ensures that staff provides supportive services to participants as outlined in the plan of care.
  • Ensures that staff maintain participants’ medical record and fulfills St. Paul’s PACE charting and reporting requirements.
  • Ensure staff provide timely notification to the Clinical Services Manager, or designee, of any marked change in the participant’s condition
  • Assist with supply management and ordering; manage home care inventory and distribution across sites.
  • In collaboration with the In-Home Services CNA supervisor, oversees in the coordination of personal care activities of all participants in the home.
  • Monitor and complete required data entry in Axis Care for participants receiving In-Home Services.
  • Maintains overall quality and fiscal accountability for outside vendor services. Approve and process vendor in-voices in a timely manner
  • Participates in developing and organizing new staff orientation, training and ongoing in-services training for staff, independent contracts.
  • Leads all department meetings and develops agendas in collaboration with In Home services CNA supervisor(s).
  • Collaborates with other nursing leaders in the organization and Center Directors on best practices and in-home services policies and program development
  • Support quality improvement initiatives and participate in projects to enhance care delivery and operational efficiency.
  • Participates in and supports Quality Improvement initiatives, attending meetings regularly to monitor and promote ongoing department improvements.
  • Educates participants, family and caregivers in regard to in-home care services. Responds to grievances within a timely manner.
  • Follows all St. Paul’s PACE Policies and Procedures and OSHA safety guidelines.
  • Maintains the confidentiality of all company procedures, results, and information about participants, clients or families.
  • Practices Universal precautions and follows appropriate Infection Control procedures.
  • Maintains safe working environment. Follows St. Paul’s PACE Care Safety Policies and Procedures and Universal Precautions
  • Participates in continuing education classes and any required staff and training meetings.
  • Performs other duties as required or requested.
