About The Position
A Home Care Registered Nurse (RN) provides skilled nursing care to patients in their homes, promoting independence, comfort, and optimal health outcomes. The RN works closely with patients, families, and healthcare teams to develop and implement individualized care plans.
Requirements
- Active Registered Nurse (RN) license in the state of practice
- Graduate of an accredited nursing program (Associate or Bachelor’s degree)
- Current CPR/BLS certification
- Minimum of 1–2 years of clinical nursing experience
- Strong assessment, communication, and organizational skills
- Ability to work independently and manage time effectively
- Valid driver’s license and reliable transportation
Nice To Haves
- home health experience preferred
Responsibilities
- Assess patients’ physical, emotional, and environmental needs in the home setting
- Develop, implement, and update individualized care plans
- Administer medications, treatments, and wound care as prescribed
- Monitor patient progress and document all care provided accurately
- Educate patients and family members on disease management, medications, and home care techniques
- Coordinate care with physicians, therapists, and other healthcare professionals
- Recognize changes in patient condition and respond appropriately
- Ensure compliance with healthcare regulations, policies, and safety standards
