About The Position
Community Care is hiring a Registered Nurse for its Family Care Program in Milwaukee, Wisconsin. The role supports older adults and adults with physical and intellectual disabilities in living as independently as possible in the community. The RN collaborates with a team to provide community nursing case management and member-centered care coordination. The position includes regular local travel using a personal vehicle. It is a Monday through Friday day-shift position, 8:00 a.m. to 4:30 p.m. A $10,000 sign-on bonus is offered.
Requirements
- Associate's degree in Nursing.
- Wisconsin Registered Nurse (RN) license.
- At least 3 years of recent nursing experience with older adults or people with physical or intellectual disabilities.
- Valid driver's license and proof of required personal vehicle insurance.
- Eligibility for long-term care functional screener certification.
Nice To Haves
- Bachelor's degree in Human Services or a closely related field.
- Current long-term care functional screener certification.
- Three years of community nursing experience.
Responsibilities
- Manage a caseload of Family Care Program members.
- Complete comprehensive clinical assessments.
- Collaborate to develop member-centered care plans.
- Provide ongoing care coordination, member education, quality assurance, and service monitoring.
- Travel locally using a personal vehicle.
