About The Position

The Care Manager RN at Providence Queen of the Valley Medical Center in Napa, California, carries out effective case management for identified patients. The role plans, coordinates, negotiates, procures, and manages care and other needs for complex patients and their families throughout the continuum of care, and helps patients use appropriate health care services. Depending on the inpatient setting, the position may include utilization management, discharge planning, and chronic disease case management. Requirements

Complete a recent post-graduate-level curriculum focused on case management, or have current clinical experience with two years of clinical experience in an acute care hospital, or one year of current experience in an inpatient or outpatient case management setting.
Hold a California Registered Nurse License upon hire.
Have two years of case management, discharge planning, or utilization management experience in an acute, sub-acute, home health, or managed care setting.

Nice To Haves

Bachelor’s degree in Nursing; required within three years of hire.
Three years of acute care experience.
