The RN Care Manager will conduct complete assessments, establish appropriate plans, and initiate interventions within desired timeframes. This role involves effective collaboration and negotiation with patients, families, and the healthcare team to achieve patient and organizational goals related to care needs, choices, and satisfaction during discharge planning and care transitions. The Care Manager will utilize patient/family strengths in problem-solving, involving them in the decision-making process from admission throughout the hospital stay. They will provide continuity of care and discharge planning services compliant with regulatory standards by offering coordinated options and services based on assessed needs, ensuring patients, families, and the healthcare team are informed and able to proceed with accountabilities. This includes facilitating smooth transitions for patients, families, and staff during transfers. The role also encompasses providing case management services for various levels of healthcare, finances, housing, family discord, or illness adjustment, managing family dynamics and crisis situations professionally, utilizing community resources effectively, and educating patients/families on accessing and using services. The RN Care Manager will initiate internal and external referrals, document discharge planning interventions and utilization review activity per standards, and perform accurate concurrent and retrospective reviews. Effective communication with the healthcare team, partnership with Social Work and unlicensed personnel, and active participation in the Outcome Facilitation Team/Patient Care Multidisciplinary Team are essential. This role requires collaboration with managers, physicians, medical directors, and treatment teams to address barriers to discharge, expedite care delivery, and implement/report care coordination, discharge planning, and utilization management activities. The RN Care Manager will refer cases to a physician advisor as needed and remain knowledgeable about healthcare regulations, reimbursement, length of stay impacts, and community resources. They will complete UM activities, including providing clinical updates to payers, collecting data, supporting UM activity, and managing avoidable delays. Delivering CMS regulatory notices within established timeframes and developing productive relationships with community-based agencies are also key responsibilities. Collaboration with Advocate Aurora Ambulatory Care Management and Continuing Health is expected to meet common goals. The RN Care Manager will serve as an educator and expert resource to medical and hospital staff regarding admission status, acute care criteria, UM issues, care coordination, discharge planning, and regulatory requirements. The role requires demonstrating knowledge and skills for age-appropriate patient care, understanding growth and development principles, and assessing/interpreting patient status based on age-specific needs.
