About The Position

Shriners Children’s is seeking an RN Care Manager to provide assessments, planning, interventions, and follow-up for patients and their families across the continuum of care. Requirements

BSN or in progress
RN license in state of practice
Case Management Certification (CMC, ACM or ANCC), or willingness to achieve within 12 months of eligibility.
3 years Nursing experience

Nice To Haves

MSN
5+ years of clinical experience in Pediatrics and/or Orthopaedics
3-5 years Care Coordination/Case Management experience

Responsibilities

Coordinates the plan of care to achieve desired outcomes and integrates the work of the health care team.
Ensures the development of a safe and effective plan of care through early identification and assessment of the patient's needs and the resources available.
Collaborates with patients, families, caregivers, and the health care team locally and within the SHC system to jointly communicate, problem solve, and share accountability for optimal outcomes.
Arranges services among community agencies, physicians, patients, family, caregivers, and others involved in the plan of care.
Ensures prudent utilization of all resources by evaluation of the resources available to the patient and balancing cost and quality to ensure optimal clinical and financial outcomes.
