About The Position

The Home-Based Primary Care program is seeking a Registered Nurse (RN) to join an innovative team caring for homebound adult/geriatric patients. The RN will collaborate with an interdisciplinary team including MDs, APPs, Social Workers, Schedulers, and other RN Care Coordination (RNCC) nurses. The primary duties involve care coordination, triage, and case management. Patient and family communication will be conducted via telephone or video, with no in-home visits currently. The role requires utilizing nursing skills for triaging emergent issues, providing resources, initiating/facilitating consults/referrals, updating care plans, and coordinating care with rehab/LTC facilities.

Requirements

  • Graduate of school of nursing
  • 6 months RN experience
  • Registration as a registered nurse in state where employed or licensure in the state licensure compact Upon Hire
  • CPR/BLS Upon Hire

Nice To Haves

  • Bachelor's Degree in Nursing or higher
  • Previous experience in adult acute care or ambulatory setting
  • Previous Case management experience
  • Case Management Certification
  • 3-5 years of adult acute care or primary care nursing experience

Responsibilities

  • Collaborate with a focused interdisciplinary team of MDs, APPs, Social Workers, Schedulers and other RN Care Coordination (RNCC) nurses to care for patients enrolled in the program.
  • Perform the main duties of the RNCC nurse that include care coordination, triage and case management.
  • Communication with patients/families is via telephone or video communication (no in-home visits at this time).
  • Utilize nursing skills such as triaging emergent issues, providing resources to clients and their families, initiate/facilitate consults/referrals, update care plans, and work with rehab/LTC facilities to coordinate care.
