About The Position

Join the innovative Home-Based Primary Care program working with the adult/geriatric population that are homebound. We are seeking a Registered Nurse (RN) to collaborate with a focused interdisciplinary team of MDs, APPs, Social Workers, Schedulers and other RN Care Coordination (RNCC) nurses to care for patients enrolled in the program. Perform the main duties of the RNCC nurse that include care coordination, triage and case management. Communication with patients/families is via telephone or video communication (no in-home visits at this time). Utilize your nursing skills such as triaging emergent issues, providing resources to clients and their families, initiate/facilitate consults/referrals, update care plans, and work with rehab/LTC facilities to coordinate care.

Requirements

  • Graduate of school of nursing Required
  • 6 months RN experience Required
  • Registration as a registered nurse in state where employed or licensure in the state licensure compact Upon Hire Required
  • CPR/BLS Upon Hire Required

Nice To Haves

  • It is preferred that nurses have 3-5 years of adult acute care or primary care nursing experience.
  • Bachelor's Degree in Nursing or higher Preferred
  • Previous experience in adult acute care or ambulatory setting Preferred
  • Previous Case management experience Preferred
  • Case Management Certification Preferred
