This role is a W2 opportunity, with care being provided at the patient’s residence in the Espanola Valley of New Mexico. The RN in this position will play a vital role in supporting clinical operations by bridging staffing gaps in unassigned homes and working directly with patients and families to ensure acceptance and utilization of authorized care. This role ensures continuity of care, promotes patient engagement, and supports compliance with clinical and regulatory standards. When not actively filling staffing gaps, the RN Bridge Nurse contributes to clinical oversight through supervisory visits, documentation review, and nurse education. In addition to clinical responsibilities, the RN Bridge Nurse collaborates closely with the Operations team — including Care Team Directors, Staffing, and the Executive Director — to align field staffing strategies with patient needs. While this position reports directly to the DON, the RN Bridge Nurse maintains a collaborative (dotted-line) relationship with the Executive Director to ensure coordination between clinical and operational priorities. Requirements

  • Active Registered Nurse (RN) license in the state of practice (BSN preferred).
  • 3+ years of nursing experience; home health or transitional care experience preferred.
  • Strong communication and interpersonal skills, with a focus on patient-centered care.
  • Knowledge of DOL/EEOICPA guidelines and home health regulations a plus.
  • Proficient in EMR systems; experience with IGEA preferred.

Responsibilities

  • Provide direct nursing care in homes without assigned staff to maintain continuity and quality of care.
  • Complete assessments and documentation (e.g., T1001) for patients in unstaffed homes.
  • Identify and report trends in staffing gaps and collaborate with the DON and Care Team Directors to address them.
  • Support interim care planning and execution until permanent staffing is secured.
  • Engage patients and families to encourage acceptance of approved care services.
  • Educate on the importance and benefits of home health services.
  • Participate in care plan discussions and family meetings to resolve concerns or hesitations.
  • Collaborate with Patient Coordinators and Directors to align care delivery with patient needs.
  • Conduct supervisory visits for patients without assigned nurses or those assigned to LPNs.
  • Review and sign Case Management notes to ensure compliance and quality.
  • Support nurse education and mentorship to improve documentation, clinical judgment, and utilization practices.
  • Notify physicians and interdisciplinary teams of changes in patient condition.
  • Serve as a clinical resource for field staff and leadership.
