Requirements

  • Must be a graduate of an approved school of nursing, therapy, or social work.
  • Must be licensed in the state where they currently practice.
  • Must have two years' demonstrated clinical experience.
  • Must have demonstrated experience and understand federal, state, and local laws and regulatory guidelines governing the operations of a home health or hospice.
  • Must have basic demonstrated technology skills, including operation of a mobile device.
  • Must possess a valid state driver license
  • Must maintain automobile liability insurance as required by law
  • Must maintain dependable transportation in good working condition
  • Must be able to safely drive an automobile in all types of weather conditions

Nice To Haves

  • Three years' demonstrated clinical experience is preferred.
  • Previous experience in and knowledge of home health and hospice practices is strongly preferred.
  • Demonstrated experience and understanding of customer and market dynamics, as well as the requirements and best practices of transitional care within home health or hospice is preferred.

Responsibilities

  • Represent the organization in internal transitional care activities utilizing standardized processes for identifying, engaging, and transitioning patients with life limiting illnesses who are eligible for the hospice benefit.
  • Integrate clinical guidelines, tools, and other metrics in the development of transition plans that are patient centered, promoting quality and efficiency in the delivery of care.
  • Serve as a representative to bring awareness and internal education relative to hospice services available, especially the care connections program.
