About The Position

This position is for a Registered Nurse (RN) in Ambulatory Nephrology, working 90% FTE on day shifts, Monday through Friday, 8:00 am to 5:00 pm, with one day off every other week. The role involves working at the Kidney Clinic in Fitchburg, WI, and Eastpark Medical Center in Madison, WI. The RN will provide evidence-based, high-quality nursing care via telephone and MyChart, assist with care coordination, patient education, prior authorizations, and support the Polycystic Kidney Disease (PKD) Center of Excellence. Responsibilities include managing new patient referrals, triage, and assisting with transitions to dialysis in a fast-paced, team-oriented environment. The role requires critical thinking skills and educating Chronic Kidney Disease patients on managing their condition to delay progression. The RN will work as part of a multidisciplinary team to ensure consistent and positive patient outcomes, utilizing the Nursing Process for assessment and clinical decision-making.

Requirements

  • Graduate of school of nursing Required
  • 6 months of RN experience Required
  • Registration as a registered nurse in the state where employed or licensure in a state in the licensure compact Upon Hire Required
  • CPR/BLS Certification Upon Hire Required

  • Relevant RN experience Preferred
  • Applicable clinical certification Preferred

Responsibilities
