About The Position

This is a 90% FTE, day shift position, Monday – Friday, 8:00am – 5:00pm with one day off every other week. The role is located at the Kidney Clinic in Fitchburg, WI and Eastpark Medical Center in Madison, WI. The Registered Nurse (RN) will provide evidence-based, high-quality nursing care for patients via telephone and MyChart. Responsibilities include assisting with care coordination, patient education, facilitating prior authorization, and other duties supporting the general Nephrology and Polycystic Kidney Disease (PKD) Center of Excellence. The role involves working in a fast-paced, team-oriented environment, managing tasks such as new patient referrals, triage, and assisting with the transition to dialysis. The nurse will utilize critical thinking skills daily and educate Chronic Kidney Disease patients to help them understand their disease process and how to delay progression. This position requires working as part of a multidisciplinary team for consistent and positive patient outcomes, determining patient care needs through the Nursing Process as a framework for assessment and clinical decision-making.

Requirements

  • Graduate of school of nursing
  • 6 months of RN experience
  • Registration as a registered nurse in the state where employed or licensure in a state in the licensure compact Upon Hire
  • CPR/BLS Certification Upon Hire

  • Applicable clinical certification
