This is a remote position supporting the SE market, working Monday through Friday, 40 hours per week. The role involves providing language-specific education, psychosocial support, and necessary resources for newly diagnosed cardiac imaging patients and their families. The Nurse Navigator will coordinate appropriate referrals for needed services, educate patients on diagnosis, work-up, treatments (including surgery), therapy, support services, financial issues, community resources, survivorship, and follow-up care. They will serve as a liaison between departments for patients and families, coordinate new patient referrals, schedule exams, procedures, and appointments, identify potential gaps in care, and coordinate care for patients returning to their communities. The Navigator will contact every newly diagnosed patient at the time of diagnosis, assess their emotional and social needs, identify barriers to care (such as health insurance and transportation), and refer them to necessary resources. Additionally, the role involves identifying trend barriers to patient care and conducting service recovery to ensure patient satisfaction.
