About The Position

This role involves serving as an Admission Care Liaison, functioning as a Registered Nurse. The position is full-time and requires the nurse to be compassionate and experienced in hospice admissions, patient assessments, and care coordination. Responsibilities include evaluating patient needs, educating families on hospice services, developing care plans, and collaborating with healthcare professionals to ensure high-quality end-of-life care. The role emphasizes patient-centered care, quality improvement, and teamwork.

Requirements

  • Graduate of an accredited nursing school
  • Registered Nurse (RN) license in the state of Ohio without any board actions
  • CPR Certified
  • Computer skills sufficient to properly document services and care.
  • Valid driver's license and acceptable driving history.
  • Ability to travel as needed in varying weather and road conditions.
  • Current automobile insurance meeting state minimum requirements.
  • Ability to successfully complete a background investigation and 10-panel drug screening.

Nice To Haves

  • Two years acute care nursing experience preferred
  • Certification in Hospice and Palliative Medicine (CHPN) preferred and will be made available to Ohio's Hospice employees who wish to obtain certification.

Responsibilities

  • Conduct comprehensive patient and family assessments, addressing physical, emotional, psychosocial, and spiritual needs in hospitals, long-term care facilities, assisted living communities, and home settings.
  • Educate patients and families about hospice services and eligibility, supporting informed decision-making and smooth admissions processes.
  • Collaborate with interdisciplinary team members to create and initiate individualized plans of care that align with patient goals and needs.
  • Communicate clinical findings and patient status to hospice physicians to support eligibility determination and certification.
  • Provide ongoing assessments, symptom management, and care coordination for patients in contracted facilities while facilitating transitions between care settings as needed.
  • Partner with physicians, hospital personnel, and care teams to ensure continuity of care and timely discharge planning consistent with hospice principles.
  • Support organizational operations and patient care initiatives by assisting across departments when needed.
  • Participate in quality improvement activities to enhance patient outcomes and service excellence.
  • Mentor and orient newly hired staff, fostering teamwork, professional development, and positive working relationships.
