About The Position

This role is designed for nurses who thrive on solving complex clinical puzzles, performing comprehensive assessments, and creating the plan that guides a patient's entire episode of care. As an Admissions RN, you will focus on Start of Care visits, using your assessment skills, clinical judgment, and OASIS expertise to ensure every patient begins their home health journey with confidence. This opportunity allows for a focus on comprehensive admissions instead of managing a large ongoing caseload, practicing at the top of your license, and enjoying the flexibility of managing your own daily schedule and territory. You will become the clinical expert who lays the foundation for outstanding patient outcomes and earn exceptional per-visit compensation while working independently.

Requirements

  • Active Minnesota Registered Nurse (RN) license.
  • Strong clinical assessment and critical thinking skills.
  • Excellent communication and organizational abilities.
  • Ability to work independently in a field-based role.
  • Reliable transportation.

Nice To Haves

  • Home health and OASIS experience preferred.
  • Military nursing experience from Army 66H/66N, Navy 1960, Air Force 46FX, Marine Corps medical specialties, and Coast Guard HS is highly valued.

Responsibilities

  • Complete comprehensive Start of Care (SOC) assessments in patients' homes.
  • Accurately complete OASIS documentation and establish individualized plans of care.
  • Educate patients and caregivers about diagnoses, medications, and home health services.
  • Coordinate care with physicians, therapists, case managers, and other members of the care team.
  • Ensure smooth transitions from hospital, rehabilitation, or skilled nursing facilities to home.
  • Maintain timely and accurate documentation.
