About The Position
Phoenix Home Care & Hospice is a mission-driven company committed to supporting its employees. As a Phoenix Admissions RN, you will be the first point of contact between a patient and hospital. The admission RN should possess strong skills in physical assessment, diagnostic interpretation, and communication to formulate an individualized plan of care according to physician orders that incorporates the analysis of initial assessment. You will work alongside other nurses, Physical Therapists, Speech Therapists, and Occupational Therapists.
Requirements
- Missouri or Compact Nursing License
- Strong organizational and self-management skills
- Valid driver’s license
Responsibilities
- Skilled Visits
- Day-to-day patient interaction
- Education on Health Conditions, such as CHF, CODP, Diabetes
- Wound Care
- Medication Management
- Changes dressings, bandages and contraptions as prescribed
- Documentation of patient’s condition and pointing out significant changes, to be reported to the physician.
- Working alongside other nurses, Physical Therapist, Speech Therapist and/or Occupational Therapist.
