The Embrace Initiative at the University of Maryland, Baltimore has an exciting opportunity for a Case Manager (Community Program Specialist) who will serve at our Restoration Gardens 1 Site. The Restoration Gardens 1 Case Manager will provide comprehensive, housing-focused case management to transition-age youth who have exited foster care and are residing in a permanent supportive housing setting. The Case Manager will support participants in identifying and working toward individualized goals related to housing stability, health and behavioral health, education, employment, benefits, independent living, and overall well-being, while coordinating referrals and services with community-based partners as appropriate. The Case Manager will maintain timely and accurate case documentation, including required client, service, and outcome data in HMIS, and comply with applicable documentation and data-quality requirements. As part of the role, the Case Manager may be required to independently arrange transportation to meet participants at appointments, obtain resources or supplies for the community, and attend meetings with community partners. The Case Manager will also support socioemotional workshops, groups, and community events coordinated by the EMBRACE on-site team, contribute to program operations and reporting, and collaborate with partner agencies. This full-time, contractual position requires the ability to work evenings and weekends as necessary.
