The Embrace Initiative at the University of Maryland, Baltimore has an exciting opportunity for a Case Manager (Community Program Specialist) who will serve at our Eutaw Street Site. The Foster Youth Case Manager will provide dedicated housing navigation and follow-along case management to transition-age youth who have exited foster care and are experiencing or at risk of housing instability. The Case Manager will serve at least 25 youth annually while maintaining an active caseload, providing individualized support with housing search and placement, resource navigation, service coordination, and ongoing stabilization support to promote successful transitions to and retention of permanent housing. The Case Manager will maintain timely and accurate case documentation, including required client, service, and outcome data in HMIS, and comply with applicable documentation and data-quality requirements. Services may be provided in office, community, and other locations based on participant needs; therefore, the Case Manager will conduct visits with youth as needed and will be responsible for independently arranging transportation to and from community-based meetings and service locations. The position may also support program operations, reporting, and coordination with partner agencies. This full-time, contractual position requires the ability to work evenings and weekends as necessary.
