About The Position

The Residential Nurse will be responsible for completing health assessments, overseeing the nursing program, managing medication administration, conducting direct care nursing activities, and ensuring proper medication disposal. This role involves training and supervising the Health and Wellness Coordinator, ensuring residents receive appropriate medical attention, and maintaining compliance with regulations. The position requires availability for occasional evening or weekend work and the ability to work independently and collaboratively with other professionals.

Requirements

  • Licensed Practical Nurse (LPN) with a current license in the State of Nebraska.
  • Minimum of two years of work experience in a behavioral health setting preferred.
  • Knowledge of and interest in the treatment of substance use disorders and mental illness.
  • A minimum of two (2) years sobriety if diagnosed with a substance use disorder.
  • Competency with treatment issues specific to women.
  • Knowledgeable regarding DSM-5 and experienced in working with clients with co-occurring disorders.
  • Willing to become knowledgeable of supportive services and other referral agencies in the community.
  • Familiarity with Federal and State Regulations on 42 CFR Part 2 and HIPAA.
  • Ability to work independently with minimal supervision.
  • Ability to prepare reports for client records per agency guidelines and timelines.
  • Ability to work with other agency professionals to meet needs and best interest of consumers, serving as a liaison between agencies.
  • Current, valid driver’s license and operational vehicle that meets current state safety standards with liability insurance coverage.
  • Ability to provide proof of coverage to transport residents if necessary for referral or assistance if other staff is unavailable.

Responsibilities

  • Complete Health Assessment for each individual within 24 hours of admission, including evaluation for infectious disease, as part of the DDP/IR resident intake process.
  • Gather health information, including physician orders, medical conditions, allergies, medication history, and current health needs.
  • Formulate recommendations for further medical assessment and care, communicating them effectively.
  • Provide recommendations to the Health and Wellness Coordinator for the Individual Treatment Plan.
  • Oversee the Nursing program for the WELL through training and monitoring of Health and Wellness Coordinator activities.
  • Provide training for the Health and Wellness Coordinator regarding additional or delegated activities for the Nursing program.
  • Supervise the Health and Wellness Coordinator to ensure residents are offered medical attention when needed and arrangements for health services are made.
  • Supervise the Health and Wellness Coordinator to ensure residents have access to qualified health care professionals for monitoring and regular health screenings.
  • Supervise the Health and Wellness Coordinator to ensure residents have access to emergency medical services when needed.
  • Supervise the development, implementation, and updates of all treatment plans, with primary responsibility for medical care and education.
  • Work with the Health and Wellness Coordinator to assess individual resident needs for medical/dental/vision care, medication compliance, weight management, chronic pain, and comprehension of medical information.
  • Oversee all Medication Aide training and Medication Administration Records.
  • Oversee Medication Aide certification, including competency assessment, for all Residential Technicians.
  • Monitor and communicate resident medication administration in accordance with physician's orders.
  • Provide direction and monitoring in the provision of routine and additional activities of Medication Aides.
  • Oversee the tracking and reporting of any medication errors to the client’s licensed practitioner.
  • Confer with the Health and Wellness Coordinator and Executive Director regarding medication errors and training needs.
  • Oversee individual Medication Administration Records for each resident for accuracy and completeness.
  • Determine resident capability for self-administration of medication and the need to return to staff-administration.
  • Conduct direct care nursing activities as needed, including medically monitoring residents with chronic conditions.
  • Regularly monitor residents’ compliance in taking prescribed medications.
  • Utilize CIWA-Ar for residents with acute withdrawal symptoms and assess the need for detox.
  • Be available for after-hours and weekend on-call assignments related to medications and medical questions.
  • Confer with the Executive Director on the need for a more structured environment for individual residents.
  • Oversee the disposal of medications per agency policy and procedure.
  • Assist the Executive Director and Health and Safety Coordinator in developing and maintaining a system for management of identified infections within the facility.
