As our Residential Living Care Coordinator, you’ll connect residents with the right support, guide transitions in care, and build trusted relationships with residents, families, and team members. Messiah Lifeways is a nonprofit organization in Mechanicsburg, PA, offering a network of services that helps adults age 55 and better live with purpose in South Central Pennsylvania. Since 1896, when the Brethren in Christ Church founded Messiah Home in Harrisburg, PA, we have served older adults with Christ-like love.

Requirements

RN or LPN licensures
Proficiency in Microsoft Office Suite and Medical Records software applications
Strong Organizational Skills: Managing and coordinating multiple resident care plans and clinical needs.
Strong Communication Skills: Clear written and verbal communication with residents, their families, and other departments in the organization.

Geriatric case management or Long Term care preferred

Responsibilities

Partner with the Residential Living Administrator to identify residents who need additional support, such as Home Care, Home Health, Personal Care, or Skilled Nursing, and help put those services in place.
Assist Residential Living Administrator with follow up visits/calls with RL residents returning to their cottage/apartment from a higher level of care or acute hospital setting.
Manage the process for Residential Living monthly charges for medical services rendered.
Collaborate with RL team members on the organization and maintenance of charts and health records.
Complete designated portion of new resident group orientation for each new RL resident within two weeks of their move in.
Track and record the biennial physicals, TB screenings, and vaccinations in Electronic Health Record and file the hard copy in the resident's record.
