About The Position

Sample Supports is seeking a Residential Coordinator to oversee and support host home placements, family caregivers, and adults in supported apartment programs. This role requires excellent judgment, compassion, and follow-through. The coordinator will manage a caseload, build relationships with stakeholders, conduct home visits, ensure compliance with regulations, monitor medical needs, develop support plans, complete documentation, provide coaching to caregivers, participate in team meetings, and respond to crises. The position involves working in the community, solving problems, advocating for individuals, and collaborating with an interdisciplinary team. This role is not a traditional desk job and offers variety and autonomy.

Requirements

  • High school diploma or GED required.
  • 1–2 years of professional experience (for Level I).
  • Valid driver's license and reliable transportation.
  • Ability to obtain required certifications (CPR, First Aid, Medication Administration, NVCI).
  • Strong organization, documentation, and communication skills.
  • Bachelor's degree in Human Services, Psychology, Social Work, or related field preferred (for Level I).
  • 2–4 years of experience supporting individuals with intellectual/developmental disabilities or similar populations (for Level II).
  • Experience coordinating services with interdisciplinary teams (for Level II).
  • Demonstrated ability to independently manage a full caseload (for Level II).
  • Bachelor's degree preferred (for Level III).
  • 4+ years of residential, case management, or IDD services experience (for Level III).
  • Demonstrated success managing high-acuity or complex cases (for Level III).
  • Experience mentoring or training staff (for Level III).
  • Advanced knowledge of DDD regulations, compliance, and residential program operations (for Level III).

Nice To Haves

  • Bachelor's degree in Human Services, Psychology, Social Work, or related field preferred (for Level I).
  • Experience with trauma-informed care preferred (for Level II).
  • Ability to serve as a resource for Coordinator I & II staff and support the Residential Manager (for Level III).

Responsibilities

  • Manage a caseload of adults receiving residential services.
  • Build strong relationships with individuals, host home providers, family caregivers, guardians, and interdisciplinary teams.
  • Conduct home visits, safety inspections, welfare checks, and quality assurance reviews.
  • Ensure compliance with Medicaid, HCPF, CDPHE, and agency regulations.
  • Monitor medical needs, appointments, medication protocols, and follow-up recommendations.
  • Develop and implement person-centered support plans and behavioral interventions.
  • Complete documentation, audits, reports, and regulatory requirements accurately and on time.
  • Provide coaching, and oversight to caregivers and providers.
  • Participate in team meetings, HRC reviews, court hearings, and interdisciplinary planning.
  • Respond calmly and professionally during crises while exercising sound clinical judgment.
  • Provide direct support to individuals when operational needs require.
  • Advocate for services, resources, and quality outcomes for every person supported.
  • Respond to emergency calls from staff, families, providers, hospitals, law enforcement, and crisis teams.
  • Make time-sensitive decisions regarding health, safety, placement, and services.
  • Coordinate emergency supports when needed.
  • Occasionally respond in person when situations require.
  • Perform unannounced home visits.
  • Drive throughout your assigned caseload.
  • Cover direct support shifts when operationally necessary.
  • Provide physical intervention using NVCI techniques when trained and necessary to maintain safety.
  • Fulfill mandatory reporting responsibilities for abuse, neglect, mistreatment, and exploitation.
  • Maintain confidentiality while making independent decisions.
