About The Position

Horizon House, Inc. is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues, and substance abuse. Horizon House is Pennsylvania’s oldest psychiatric rehabilitation agency. Since 1952, we have been dedicated to helping people with mental illness, drug and alcohol addictions, developmental/intellectual disabilities, and homelessness to stabilize their lives and to help them live as independently as possible in the community. With over 100 locations throughout South and North Eastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strength and choice. Our Prospect Park, PA program is seeking a dedicated Residential Coordinator to join our team. The Residential Coordinator coordinates and provides guidance, counseling, and encouragement to clients as they transition from the mental health system to a more independent environment.

Requirements

  • Associate's degree in the Social Sciences, healthcare or a related field and 2-3 years of relevant experience in the MH/MR field; or any combination of experience, education or training that would provide the level of knowledge, skill and ability required.
  • Good communication skills, both oral and written.
  • Ability to work independently and as part of a team.
  • Some knowledge of mental health systems and supports.

Nice To Haves

  • Prior supervisory experience, helpful.

Responsibilities

  • Assisting the Manager with the daily operations of the site and supervises direct service staff.
  • Implements and documents client progress, in accordance with individualized rehabilitation service plans.
  • Providing appropriate supervision of subordinate staff including training, monitoring, performance feedback and discipline.
  • Encourages and initiates regular discussions with subordinates; compliments good performance; takes appropriate and timely action with marginal or failing performers.
  • Promotes staff development by recommending training appropriate for employees' growth and improvement needs.
  • Overseeing and coordinating the inventory of clothing and personal items regularly, to ensure that these items are maintained.
  • Maintaining and monitoring medications consistent with individuals' self-medication plans, in compliance with State and Agency medication administration regulations.
  • Providing appropriate intervention and coverage, to maintain the smooth transition from one shift to another and/or the change to day programming and then back again to the site.
