This position coordinates and oversees the implementation of active treatment programming and residential services for individuals residing in assigned areas. It also involves direct or indirect participation in the development and implementation of individual active treatment plans in cooperation with professional and para-professional staff by attending meetings and participating in evaluations. The role provides support related to Electronic Health Record (EHR) processes, including assisting staff with EHR documentation and reinforcing documentation expectations. Collaboration with interdisciplinary team members is essential to ensure continuity of care and services. The position also involves approving/disapproving leave requests, making assignments, and overseeing/monitoring the KRONOS timekeeping system.

Requirements

  • 24 months training or 24 months' experience in providing direct care to patients &/or in participating in development &/or carrying out of habilitation/treatment plans for patients who had either developmental disabilities or mental disability
  • 6 months of required training &/or experience should have been as ward charge or team leader with responsibility for ensuring other workers performed their duties & ensuring safety of sanitation of assigned work area

Responsibilities
