About The Position

This position is responsible for: Provides supervision, monitoring, reporting, and technical support services to clients in a residential facility, maintains appropriate records in addition to ongoing written and verbal communication with the supervisor in relationship to daily client activities; facilitates an emotional and physical environment conducive to enhancing positive client morale and stabilization.

  • High School Diploma and one year of related work experience preferably in a social service setting, Or Any equivalent combination of training and experience which provides the required knowledge, skills, and abilities.

Responsibilities

  • Completes a daily contact sheet on each resident (including alcohol test).
  • Performs bed checks during shift.
  • Assists clients with personal needs – clothing, hygiene supplies, linens, etc.
  • Checks the facility to ensure that all doors are properly locked.
  • Ensures that all entry doors and outside gates are locked.
  • Answers a multi-line phone system and forwards calls appropriately; refers applicants to the Case Manager for screening.
  • Maintains and updates records of all program residents; completes assigned paperwork, documentation, and statistics as assigned.
  • Completes all paperwork as needed to report problems and/or concerns, emergency reports and violations of the rules and regulations and forwards reports to Case Management and Program Director.
  • Communicates all major incidents to Case Manager and Program Director in writing and orally; facilitates ongoing information exchange.
