Requirements

  • High school diploma or equivalent and at least two years of experience in human services, social services, case management, resident services, or a related field; OR Bachelor's degree in human services, social work, sociology, psychology, gerontology, or a related field with at least one year of relevant experience
  • Minimum one year of direct service experience supporting seniors and/or individuals with disabilities or special needs
  • Valid driver's license meeting CHP insurability requirements
  • Microsoft Office Suite (Word, Excel, Outlook, PowerPoint)
  • Virtual meeting platforms including Teams and Zoom
  • Online benefits and government service systems
  • Documentation, data entry, and reporting tools

Responsibilities

  • Conducting resident assessments and developing individualized support plans
  • Connecting residents with healthcare, benefits, transportation, food assistance, and other essential services
  • Planning and facilitating wellness, educational, and social programs
  • Building partnerships with community organizations and service providers
  • Assisting residents with benefit applications and service navigation
  • Maintaining accurate documentation, reporting, and compliance requirements
  • Managing program resources and supporting community operations
  • Collaborating with property management and internal teams to improve resident outcomes
