About The Position

Community Housing Partners (CHP) is seeking a passionate, resourceful, and relationship-focused Senior Services Coordinator to help older adults and individuals with special needs access the resources, support, and community connections they need to thrive. This role involves being a trusted advocate, community builder, and strategic partner who helps residents maintain stability, improve wellness, and remain successfully housed. The position offers the opportunity to make a meaningful difference in the lives of vulnerable populations while building partnerships that strengthen entire communities. The role focuses on building meaningful relationships, creating engaging communities through programs that promote wellness and social connection, and expanding professional expertise in resident services, community engagement, benefits navigation, partnership development, program management, and outcomes reporting. Requirements

  • High school diploma or equivalent and at least two years of experience in human services, social services, case management, resident services, or a related field; OR Bachelor's degree in human services, social work, sociology, psychology, gerontology, or a related field with at least one year of relevant experience
  • Minimum one year of direct service experience supporting seniors and/or individuals with disabilities or special needs
  • Valid driver's license meeting CHP insurability requirements
  • Microsoft Office Suite (Word, Excel, Outlook, PowerPoint)
  • Virtual meeting platforms including Teams and Zoom
  • Online benefits and government service systems
  • Documentation, data entry, and reporting tools

  • Experience with program development and community engagement
  • Experience in affordable housing, senior services, healthcare, or nonprofit settings
  • Experience with data tracking and reporting systems such as Smartsheet

Responsibilities

  • Conduct resident assessments and developing individualized support plans
  • Connect residents with healthcare, benefits, transportation, food assistance, and other essential services
  • Plan and facilitate wellness, educational, and social programs
  • Build partnerships with community organizations and service providers
  • Assist residents with benefit applications and service navigation
  • Maintain accurate documentation, reporting, and compliance requirements
  • Manage program resources and supporting community operations
  • Collaborate with property management and internal teams to improve resident outcomes
