This position is for a full-time Resident Health Services Coordinator position to work at Camellia Place and The Oaks, a 112-unit and 36-unit affordable family housing communities in Dublin and Walnut Creek, CA. Assists residents in achieving housing stability, improved health outcomes, and overall wellbeing. Engages residents with culturally responsive service navigation, health-related support, and community-building activities within EAH Housing communities. Connects residents to existing social services, examines needs for on-site services, and explores ways to bring programs to the property, serving as a bridge between residents, property management, service providers, Managed Care Plans (MCPs), and community-based organizations. This position is characterized by a high degree of initiative, accountability, and ability to work congenially with a wide variety of individuals within EAH and outside of the organization. Understands and demonstrates commitment to the Mission and Values of EAH Housing.
