About The Position

The Resident Care Coordinator is responsible for documenting resident status, needs, and appointments, and ensuring proper documentation of care plans and nurses' notes. This role requires strong experience as a Medication Aide, providing orientation and training to caregivers and medication aides, and assisting residents with daily living activities. The coordinator will also manage wellness clinics, conduct audits, manage clinical staff scheduling, and maintain a positive and confidential environment for residents, adhering to HIPAA guidelines. Ongoing training and other assigned duties are also part of the role.

Requirements

  • High-School diploma or equivalent
  • Must be at least 18 years of age
  • Must have First Aid & CPR Certification upon hire, or receive within 30 days
  • Must have a Food Handlers Card, or receive within 30 days
  • Must have at least 1 year of strong caregiver or medication aide experience in senior living

Nice To Haves

  • Certified Nursing Assistant preferred

Responsibilities

  • Document information pertaining to changes in residents’ status, needs and appointments
  • Ensure proper documentation of all care plans, nurses notes and all other resident correspondence
  • Provide orientation and on the job training to Caregivers and Medication Aides
  • Assist with daily living activities (i.e., bathing, dressing, and eating)
  • Measure, record and assist with wellness clinics
  • Conduct weekly, monthly, and quarterly audits
  • Manage scheduling of clinical staff
  • Create and maintain an atmosphere of warmth, patience, enthusiasm, calm, and joy
  • Maintain confidentiality of all resident care in accordance with HIPAA guidelines
  • Complete on-going training as assigned
  • Complete other duties as assigned
