The Resident Care Coordinator acts as a liaison between residents, families, and the care team, fostering a supportive and safe living environment. This role supports the Health and Wellness Director by assisting with scheduling, supervising staff, and ensuring quality assisted living services. Key duties include resolving resident concerns, coordinating care with internal teams and external providers, and assisting with medication administration and direct resident care when needed.

Requirements

  • High School Diploma or equivalent.
  • Must have a valid state medication administration license (such as CNA/CMA/CMT/QMAP) or meet the state requirements for medication administration in assisted living.
  • Current First Aid and CPR license or ability to obtain.
  • At least two (2) years of experience administering medications and providing care in a professional caregiving setting is required.
  • Excellent attention to detail and high integrity.
  • Excellent communication skills and teamwork mindset.
  • Strong leadership and team management skills.
  • A positive attitude, empathy, patience, and a commitment to treating our residents with dignity and respect.

Nice To Haves

  • An LPN/LVN nursing license is a plus.
  • At least one (1) year of relevant supervisory experience, including staff scheduling, is preferred.
  • Experience with memory care is preferred.
  • Willingness to participate in weekend rotation.

Responsibilities

  • Schedule and assign caregiving and medication tech shifts to ensure adequate coverage and compliance with staffing guidelines and department allowances.
  • Participate in the recruitment and interview process for new care team members.
  • Supervise, train, and support care team members, promoting teamwork and adherence to care standards.
  • Supervise the medication room and orders, collaborating with medication technicians and caregivers.
  • Maintain accurate and up-to-date resident records, including care plans, incident reports, and medical documentation.
  • Coordinate care planning with on-site home health agencies, working within community and company policies.
  • Coordinate admissions with the sales team, including supervising move-ins.
  • Complete family/resident admission interviews.
  • Facilitate effective communication among residents, their families, and healthcare providers to ensure seamless care delivery.
  • Address residents’ special needs, requests, and complaints.
  • Assist with medication administration and provide direct care to residents as needed.
  • Understand the community's care regulations, as well as state-specific regulations.
