About The Position

This position assists in the planning, organizing, and implementation of programs for residents at different functional levels. The role involves one-to-one interaction with residents to provide companionship, sensory stimulation, reality orientation, and individual instruction on leisure pursuits, all while meeting the needs outlined in their care plans. The assistant will assess and report resident progress, maintain records of activities and visits, and manage departmental supplies and equipment. Safety supervision during activities, transportation of residents, and participation in resident care plan meetings are key responsibilities. The role also includes participating in fundraising for the Resident Recreation Fund and recognizing/reporting any abnormal signs or symptoms in residents. A significant aspect of the role involves understanding and participating in Quality Assurance and Performance Improvement (QAPI) efforts, including attending training, carrying out assigned QAPI responsibilities, and supporting QAPI initiatives through actions and attitude. Additionally, the position requires promoting a culture of safety, empowerment, and healing in line with Trauma Informed Care principles, and adhering to compliance and ethics standards by participating in annual training and reporting concerns.

Requirements

High school graduate or GED equivalency preferred.
Must have a pleasant and outgoing personality with a flair for creativity.
Must have a basic understanding of the needs of the elderly.
Must have patience and empathy.
Ability to treat Rest Haven and Resident information as confidential.
A responsible, confidential, professional, and team player.
Must be organized and an effective communicator.
Must be able to work without constant and direct supervision.
Adequate physical health to perform the essential functions.
Criminal history background clearance: Act 169 and Act 13.

Nice To Haves

Previous experience in a nursing facility is preferred.

Responsibilities

Assists in planning, organizing, and implementation of programs for residents at different functional levels.
Visits with residents on a one-to-one basis for the purpose of providing a friendly visitor, sensory stimulation, reality orientation, and individual instruction on specific leisure pursuits.
Meets the needs of residents as outlined in the care plan.
Assesses and reports resident progress.
Maintains resident written records of activities attended and 1:1 visits.
Maintains and organizes an inventory of departmental supplies and equipment.
Supervises residents during activities to assure safety per regulation and policy.
Transports residents to and from activities.
Participates in resident care plan meetings.
Participates in fundraising for the Resident Recreation Fund.
Recognizes and reports abnormal signs and symptoms of residents which may signify a physical change in their condition.
Recognizes role as part of QAPI efforts, attends trainings, carries out QAPI role and responsibilities, follows policies and procedures in support of QAPI, and shares observations on QAPI project outcomes or barriers.
Supports QAPI efforts verbally and non-verbally.
Performs other duties as assigned by Supervisor.
Promotes a culture of safety, empowerment and healing as part of Trauma Informed Care.
Protects trauma survivors from being re-traumatized.
Participates in annual compliance and ethics training.
Follows code of conduct reporting any compliance and ethics concerns.
