About The Position

Concerto Renal Services is seeking a PRN Renal Dietitian to provide nutrition education, counseling, and interventions to patients with end-stage renal disease (ESRD) and their authorized caregivers in clinical settings. The role focuses on maximizing patients’ nutritional well-being through individualized nutrition care and collaboration with the interdisciplinary team. The position is per diem in the Greater Peoria Area, Illinois, and includes a minimum of two monthly shift pickups.

Requirements

Bachelor’s degree in Nutrition, Dietetics, or a similar area.
Registered Dietitian credential and at least 12 months of clinical experience, as required by CMS.
State licensure or certification if required and available in the state where the facility is located.
Intermediate computer skills and proficiency with MS Word, Excel, and Outlook.
Proficiency with Concerto-specific applications within 60 days of employment.

Responsibilities

Conduct initial and ongoing nutrition assessments, develop individualized care plans, and update them in collaboration with the interdisciplinary team.
Design and adjust nutrition interventions based on laboratory results, dialysis adequacy, and patient goals.
Monitor patient progress and document it monthly.
Provide tailored nutrition education using appropriate materials and taking each patient’s medical, social, and economic background into account.
Participate in care conferences and quality improvement meetings.
Travel to assigned facilities and provide coverage as needed.
Maintain credentials, stay current with clinical practices, and comply with CMS regulations and company standards.
