About The Position

The Rehabilitation Specialist coordinates activities for residents of care and treatment facilities. The role assesses residents’ needs and designs and implements rehabilitation programs. It includes providing skill training, cognitive remediation, job coaching, and behavior management in accordance with treatment plans, and reinforcing appropriate behavior and skills in residential, community, and day/vocational settings. The specialist also supports therapeutic programming, prepares clinical documentation, and arranges or provides transportation to planned community activities.

Requirements

High school diploma and three years of rehabilitation experience; or an associate’s degree, technical certification, or licensure and one year of related experience; or a bachelor’s degree in a related field with no experience required.
Valid driver’s license, current vehicle registration, and insurance.

Responsibilities

Coordinate activities for residents of care and treatment facilities.
Assess resident needs and design and implement rehabilitation programs.
Perform skill training, cognitive remediation, job coaching, and behavior management objectives in accordance with treatment plans.
Reinforce appropriate behavior and skills in residential, community, and day/vocational settings using direct feedback, role-playing, modeling, and behavior modification techniques.
Assist with planning, scheduling, implementation, and evaluation of social, emotional, recreational, vocational, and related therapeutic programs for individuals served.
Prepare communication books, treatment plans, and other clinical documentation.
Arrange transportation for, or transport, individuals served to planned community activities.
