About The Position

The Rehabilitation Specialist works in the Segue Residential Transition Program, which supports individuals transitioning out of state psychiatric hospitals. Clients are clinically stable and ready to leave the hospital. Program services help clients manage their illnesses and develop independent living skills. This is a direct-care role providing support for mental health treatment and helping maintain a safe, therapeutic environment that encourages clients’ independence and wellbeing. Staff receive thorough training and supportive mentorship from senior staff members.

Requirements

• High school diploma or GED.
• Willingness to obtain additional certifications and training as required.
• Driver’s license with 3 points or less.
• Access to an insured vehicle.
• Availability to work days, nights, weekends, and holidays depending on program needs.

• Bachelor’s degree in a related field.

Responsibilities

• Participate in the initial assessment, intervention, and re-evaluation of client needs.
• Work one-on-one with clients to help them learn or regain skills for successful community and independent living and socialization.
• Coordinate comprehensive care for assigned clients, including monitoring compliance with medication.
• Provide transportation to clients’ medical appointments and attend those appointments with them.
• Advocate for clients during appointments and ensure follow-up instructions are documented and/or followed through.
• Develop rapport with clients and support them as they progress through the program and recovery.
