About The Position

The Rehab Registered Charge Nurse (RN) is the charge nurse for a 17-bed short-term rehabilitation household at Trillium Place. The role assists the RN Manager/RN Supervisor in ensuring nursing procedures are addressed and resident care issues are handled according to the Philosophy of Care, TANDEM LIVING Core Values and Mission Statement, and applicable regulations. The position is full-time, day shift, Monday through Friday, from 6:30 AM to 3:00 PM.

Requirements

High school diploma or equivalent.
Current Pennsylvania license to practice as a Registered Nurse.
Two to four years of related experience and/or training, preferably in a skilled nursing or retirement facility.
Basic computer proficiency.
Comfort using Microsoft Office, including Word and Outlook.
Current CPR certification and ability to maintain it.

Nice To Haves

Experience working with rehabilitation residents.

Responsibilities

Manage medications, evaluate vital signs, complete treatments, and change dressings.
Monitor indwelling catheters and peripheral IVs.
Lift, turn, and transfer residents as scheduled or as needed.
Use mechanical lifts properly and require two staff members for all transfers.
Monitor, respond to, and report changes in residents’ health.
Monitor residents’ skin condition and implement appropriate procedures for skin tears and pressure ulcers.
Assist in emergency situations.
Document physician visits, inputs and outputs, pharmacy requisitions, and daily charting.
Support the RN Nurse Manager in overseeing caregivers’ routine duties and serve as a resource for caregivers.
