Rehabilitation Intake Coordinator - Flower Hospital - Rehabilitation
About The Position
The Transition Coordinator manages referrals and coordinates admissions for the transition of patients to and from the Flower Rehab Center. The role includes reviewing medical records and interacting with internal and external physicians, nursing staff, therapists, leadership, coding and clinical documentation specialists, patients, caregivers, and the community. The coordinator collaborates with the FRC PPS Coordinator, Case Managers, Transition Care Managers, and Nursing and Therapy Leadership to manage insurance precertification, recertification, authorizations and appeals, length of stay, and resource utilization in a timely manner. The listed duties describe the general nature and level of the work and are not exhaustive.
Requirements
- Post-high-school clinical training equivalent to a certification or licensure.
- At least 2 years of clinical experience in a patient-care setting.
- Strong verbal and non-verbal communication skills with patients and healthcare professionals.
- Ability to work in a team environment.
- Ability to input, retrieve, and analyze relevant EMR data.
- Mid-level Microsoft Word and Excel skills.
- Reliable transportation and ability to commute between facilities.
- Ability to understand directions, communicate, and respond to inquiries using effective interpersonal skills.
- Good organizational, analytical, writing, and interpersonal skills.
- Ability to input and retrieve information using electronic devices and operate a copier, electronic device(s), printer, and scanner.
- License as indicated by education obtained.
- CPR certification and certification as indicated by education obtained.
Nice To Haves
- Associate degree in a clinical field.
- Ability to work effectively and diplomatically with a variety of internal and external customers and healthcare providers.
- 3–5 years of clinical experience in a patient-care setting.
- Previous experience with utilization review, marketing, care transitions, customer service, and/or inpatient rehabilitation.
Responsibilities
