About The Position
The RN Program Coordinator at ProMedica serves as the point person for coordinating collaborative patient care from hospital discharge through follow-up appointments. The role facilitates each patient’s plan of care, provides education and instructions for procedures or testing ordered by providers, and reviews patients’ Health Risk Assessments. The coordinator monitors patient charts for test results and reviews results with providers and patients. The role also reviews and manages preoperative surgical clearance requests with providers when needed. Other duties may include triaging patient calls and providing health advice and referrals as appropriate. This summary describes the general nature and level of work and is not exhaustive.
Requirements
- Current State license as a Registered Nurse.
- One year of relatable or equivalent experience.
- ACLS and BLS certification.
Nice To Haves
- Bachelor of Science in Nursing.
- EPIC experience.
- Two or more years of relatable experience.
