About The Position

ReMed, a Collage Rehabilitation Partner is a growing organization dedicated to excellence, honesty and ethics in the treatment of our clients and each other. For more than 35 years, we’ve developed programs to meet the diverse needs of clients with moderate to severe traumatic and acquired brain injuries. We provide treatment for post-concussion syndrome, stroke, memory loss, neurobehavioral issues and accident or sports-related head injuries. We are seeking a dedicated Rehabilitation Case Manager to coordinate and oversee comprehensive client treatment plans from admission through discharge. In this role, you will act as a primary client advocate, bridging the gap between clinical teams, families, physicians, and funding sources to ensure cohesive, high-quality care and continuous progress toward rehabilitation goals.

Requirements

  • Bachelor’s degree in a relevant healthcare or human services field required.
  • Proven experience in case management within the healthcare industry, with strong skills in crisis management, treatment plan formulation, and needs assessment.
  • Valid driver’s license and a clean driving record required.
  • Outstanding writing, organizational, time management, and inter-personal communication skills.

Nice To Haves

  • Master’s degree preferred.
  • Certified Rehabilitation Counselor (CRC) or Certified Case Manager (CCM) preferred.

Responsibilities

  • Manage assigned caseloads, lead interdisciplinary treatment team meetings, and advocate for client needs across all phases of care.
  • Monitor, evaluate, and revise individualized client rehabilitation goals in close collaboration with the Clinical Director and primary healthcare providers.
  • Maintain regular, transparent communication with clients, families, and funders regarding progress, schedule updates, and care expectations.
  • Monitor funding guidelines, facilitate alternative or future funding sources, and ensure accurate, timely documentation for billing integrity.
  • Maintain accurate client charts, oversee staff log notes, and comply with state regulatory standards and professional guidelines.
  • Provide occasional direct care support or transportation to medical appointments as needed; participate in the program’s on-call rotation.
