Skilled Nursing Facility Job Description We are seeking dedicated and compassionate individuals to join our team within a skilled nursing facility. This role is crucial in providing high-quality care to residents and ensuring a positive environment that promotes wellness and dignity.

Requirements

• High school diploma or equivalent; relevant certifications preferred.
• Experience in a skilled nursing or long-term care setting is an advantage.
• Strong interpersonal and communication skills.
• Ability to work effectively in a team-oriented environment.
• Compassionate and patient-centered approach to care.
• Current certification in CPR and First Aid is a plus.

Responsibilities

• Provide direct patient care and support to residents, addressing their individual needs and preferences.
• Assist with daily living activities, including meals, hygiene, and mobility.
• Monitor and document residents' health status and report any changes to the healthcare team.
• Administer medications and treatments as prescribed by healthcare professionals.
• Maintain a safe, clean, and therapeutic environment.
• Educate residents and their families on care procedures and healthy lifestyle choices.
• Work collaboratively with a multidisciplinary team to develop and implement individualized care plans.
