About The Position

Greenfield Rehab is seeking an experienced and dedicated Physical Therapist (PT) to serve as our Rehab Director and lead therapy services in our skilled nursing facility in Oconomowoc, WI. The Rehabilitation Director is responsible for the overall leadership, coordination, and management of the rehabilitation department within a skilled nursing facility. This role provides clinical and administrative oversight of physical, occupational, and speech therapy services while maintaining a high standard of patient care, regulatory compliance, and operational efficiency. The Rehabilitation Director also functions as a licensed Physical Therapist, providing direct patient care as needed.

Requirements

• Graduate from an accredited school with a bachelor's, master’s, or doctoral degree in Physical Therapy
• Licensed by the State of Wisconsin to practice as an Physical Therapist
• 1 year experience as a treating clinician required
