About The Position

Greenfield Rehab is seeking a full-time experienced and dedicated Occupational Therapist (OT) or Occupational Therapist Assistant (OTA) to serve as our Rehab Director and lead therapy services in our skilled nursing facility in Galesville, WI. The Rehabilitation Director is responsible for the overall leadership, coordination, and management of the rehabilitation department within a skilled nursing facility. This role provides clinical and administrative oversight of physical, occupational, and speech therapy services while maintaining a high standard of patient care, regulatory compliance, and operational efficiency. The Rehabilitation Director also functions as a licensed Occupational Therapist or Occupational Assistant, providing direct patient care as needed.

Requirements

  • Graduate of an accredited school with a bachelors, master’s, or doctoral degree in Occupational Therapy (for OT).
  • License as required by the State of Wisconsin to practice as an Occupational Therapist (for OT).
  • Graduate of an accredited school with an associate degree as a an Occupational Therapy Assistant (for OTA).
  • Certification or license as required by the State of Wisconsin to practice as Occupational Therapy Assistant (for OTA).
  • Strong clinical reasoning and patient identification skills.
  • Experience managing medical complexity and changing functional status.
  • Comfortable collaborating with nursing, therapy, and interdisciplinary teams.
  • Encourages collaboration, values others' contributions, and builds trust.
