About The Position

The Registered Practical Nurse (RPN) is a valued member of the interdisciplinary team within the department and organization. They are responsible for the coordination and delivery of quality patient care by assessing and identifying patient needs, implementing and monitoring the patients’ medical plan, and following HPHA policies and procedures. Additionally, the Registered Nurse must follow nursing processes within the profession’s and licensures scope of practice. They will provide education, advice and support to patients and family members.

Requirements

  • Current Registration and Certificate of Competence with the College of Nurses of Ontario
  • Current BLS certification
  • Completion of IV Therapy Initiation and IV Therapy Discontinuation course
  • Minimum of two (2) years recent nursing experience in an acute care setting preferred with the ability to work to full scope of practice as defined by the Huron Perth Healthcare Alliance
  • Recent clinical experience (minimum two years) in Medicine/Complex Continuing Care Nursing
  • Evidence of ongoing professional development and continuing education

Responsibilities

  • Coordination and delivery of quality patient care
  • Assessing and identifying patient needs
  • Implementing and monitoring the patients’ medical plan
  • Following HPHA policies and procedures

  • Providing education, advice and support to patients and family members
  • Ability to plan, organize and exercise effective decision making
  • Excellent critical thinking, communication and problem-solving skills
  • Ability to prioritize and function both independently and interdependently with the inter-professional care team
  • Ability to engage patient and family partners in patient education related to the Medicine/CCC setting
  • Ability to meet the physical demands of the position
  • Working knowledge of or the ability to quickly comprehend the Meditech Information System and personal computer applications
  • Knowledge of Senior Friendly Hospital Initiatives and their application in the clinical setting
  • Excellent physical assessment skills
